Helping People Recover Safely at Home

Most people want to recover and receive support in their own home whenever possible. The West Glamorgan Older People’s Programme is working with partners across health and social care to strengthen services that help people return home safely, recover well and maintain their independence following illness or a hospital stay.

Our work is built around the Home First approach and Discharge to Recover and Assess (D2RA) pathways, ensuring people receive the right support, in the right place, at the right time. This includes improving community-based recovery, assessment and rehabilitation services following discharge from hospital.

Key areas of work include:

  • Strengthening Home First and D2RA pathways across the region.
  • Improving intermediate care, reablement and community support services.

By working together across organisations, we are creating a more joined-up system that supports people to recover closer to home and remain independent for longer.
