Welcome to the 20th edition of our Joint Working Journal! This instalment is something different from the norm as the Steering and Advisory Board 1 (SAB1) meeting held on 22nd April focused solely on the work of the Communities and Older People’s Programme, which is set to undergo some significant changes in the coming months.

The meeting’s main emphasis was on hospital discharge pathways and how these could be improved to support patients who are ‘medically’ fit to leave hospital without any unnecessary delays. Board members agreed that future changes needed to incorporate the principles of the NHS Wales’s Six Goals for Urgent and Emergency Care National Programme, which aim to provide effective, high quality and sustainable healthcare as close to home as possible.

To support this transitional phase, a proposal for interim governance arrangements was brought to the Board, including the creation of a new Communities and Older People’s Programme Board which would report directly to SAB1. The Board’s membership would consist of senior representatives from all partner organisations, and we look forward to bringing you more on the progress of this vital piece of work in a future edition of our journal.


West Cross Well-being Hub is Working Wonders!

Our Communications and Engagement Team recently had the privilege of visiting the West Cross Well-being Hub. The Hub has received Regional Integration Fund (RIF) investment to help strengthen communities by providing people with a space to come together to socialise, build connections and access advice and support on a range of issues.
