Programme Key Areas

The West Glamorgan Older People’s Programme is working to transform health and social care for older people across the region, including people living with dementia and frailty.

Our aim is to help people live healthy, independent and fulfilling lives for as long as possible, in the place they call home and within the communities they know and value.

By listening to lived experience and working in partnership, we are creating services that are preventative, joined-up and sustainable for the future.

We are committed to developing innovative approaches that focus on prevention, wellbeing and self-care, helping people stay well and maintain their independence without needing more intensive support. This includes strengthening community-based services and ensuring people have access to the right help at the right time.

When people do need statutory health or social care services, we work to ensure they receive the support they need and can return home safely and quickly, with the right support in place to continue their recovery and wellbeing within their community.

Together, we are building a future where older people can thrive, remain connected to their communities, and live with dignity, choice and independence throughout later life.

Working across Swansea and Neath Port Talbot, we bring together local authorities, Swansea Bay University Health Board, third sector organisations, independent providers, older people, people living with dementia and unpaid carers to improve health, wellbeing and quality of life.

Our work focuses on three key areas:

Everything we do is shaped by partnership working, co-production and lived experience, ensuring services are designed around what matters most to older people and their families.
