This priority focuses on how statutory partners, Third Sector organisations and volunteers work collaboratively with communities. The partnership has adopted a strength-based and place-based model of prevention and community co-ordination. This supports and promotes good emotional health and well-being for individuals to remain living safe and well within their communities, without unnecessary recourse to statutory health and care services.

There is a particular emphasis on:

  • Support for carers of all ages via the the Carers’ Partnership Programme.
  • Prevention and well-being services that support the delivery of a person-centred health and care approach.
  • Expanding the range of financially sustainable accommodation-based solutions for individuals who have or may develop care and support needs, and reducing avoidable recourse to more institutionalised forms of care.
