The Housing Partnership is a regional programme established with the aim of bringing together housing, health and social care in a collaborative community.

The Housing Partnership is a regional programme established with the aim of bringing together housing, health and social care in a collaborative community. The Partnership’s work is shaped around responding to the needs of the population, with a particular emphasis on where those needs that relate to accommodation solutions.

A holistic approach across these sectors is crucial as housing is one of many factors that affect people’s health and well-being. How we plan and integrate the health and social care agenda with the housing sector is a key challenge that this programme seeks to address.

This programme is particularly relevant to people who have complex needs that require support from all three sectors (i.e. they have housing needs, health needs and care needs). This is defined as someone having ‘multiple overlapping unmet needs’ (MOUN). From here, the right collaborative ways of working can be identified to bring together the right people from housing, health, social care and other areas to address these needs, with the individual at the centre of service planning and delivery.

The main aims of the Housing Partnership are:

  • To develop a regional strategy for how housing, health and social care sectors will work together to meet the needs of our population;
  • To co-ordinate a Regional Housing Forum.
  • To plan and co-ordinate the regional capital investment to create more accommodation solutions, integrated community hubs and invest in areas such as equipment and house adaptations
  • Transform and integrate services to help people with MOUN to achieve better outcomes in a more collaborative approach across sectors.
