Programme Key Areas
The West Glamorgan Transforming Complex Care Programme is working to improve health, care and support for people with complex needs across the region. This includes children and young people, people with learning disabilities, people with mental health needs, neurodivergent people and people whose needs require coordinated support from more than one organisation.
Our goal is to help people receive the right care and support, in the right place, as close to home as possible. We want services to be easier to navigate, better coordinated and shaped around what matters to individuals, families and carers.

By bringing key areas of work together under one programme, we can better understand shared pressures, reduce duplication and develop more consistent approaches to planning, commissioning and delivering support across West Glamorgan.
The programme has a strong focus on partnership working, lived experience, evidence and data. We are committed to using this insight to understand need, identify gaps and make better decisions about future services and support.

Working across Swansea and Neath Port Talbot, our programme brings together Swansea Council, Neath Port Talbot Council, Swansea Bay University Health Board, third sector organisations, independent providers, people with lived experience and unpaid carers to improve outcomes for people with complex needs.
Our work focuses on six key areas:
- Regional Joint Working for Mental Health and Learning Disabilities
- Children and Young People with Complex Needs
- Learning Disability
- Neurodiversity
- Accommodation and Commissioning
- Market Stability, Capital and Future Planning
Everything we do is shaped by partnership working, co-production and a shared commitment to making services more consistent, sustainable and person-centred. This means improving joint commissioning, strengthening market intelligence, supporting better use of public money and helping partners plan together for current and future need.
