Learning Disability

This is a key area of the Transforming Complex Care Programme, to learn more about the programme click here.

Supporting independence, inclusion and the right care and support

The programme supports work to improve access, inclusion and wellbeing for people with learning disabilities. This includes strengthening multi-disciplinary working between health and social care, improving access to information, and increasing the number of people who receive an annual health check.

This work links closely with the regional accommodation and commissioning work, because people with learning disabilities need access to suitable, person-centred accommodation options that help them live closer to home and remain connected to their communities.

It also cross-references the regional joint working and joint funding arrangements, including pooled budgets and clearer processes for agreeing funding responsibilities, so that decisions are more consistent and people experience fewer delays in getting the right care and support.

Current priorities include:

  • Strengthening multi-disciplinary working between health and social care for people with learning disabilities.
  • Improving access to easy read information about health and social care services.
  • Increasing take-up of annual health checks for people with learning disabilities.
  • Linking with the accommodation and commissioning work to support suitable, local accommodation options.
  • Linking with joint funding and pooled budget arrangements to support clearer and more timely decisions.

The Learning Disability workstream aspires to build on previous successes and make changes where needed to transform services, enabling people with a learning disability to live fulfilling lives based on what matters to them. The intention is to ensure people with a learning disability have access to additional support in order to reach their full potential, and to eliminate barriers and inequalities.

Strategy Outcomes

Priority 1 – Transport

While developing our Learning Disability Strategy people told us that Transport is a key priority and that being able to get around to be safer, easier to use and run more often. We organised a workshop bringing together people with Learning Disabilities and professionals within the transport sector so that we can achieve this goal.
