Supporting children, young people and families through joined-up planning
This area of work supports children and young people with complex needs who require a coordinated response from health, social care, education and other partners.
The programme is working to strengthen multi-agency decision-making so that support is planned around the child or young person and their family. This includes developing a regional pathway for children and young people with complex needs.

A key aim is to help children and young people access the right support at the right time, with more local, child-centred options wherever possible.
Current priorities include:
- Developing a regional multi-agency pathway for children and young people with complex needs and therefore improving joint planning between health, social care and education.
- Working towards the Eliminate agenda by exploring options for accommodation and support closer to home.
